Limbal Stem Cell Deficiency: Current Treatment Options and Emerging Therapies
نویسندگان
چکیده
Severe ocular surface disease can result in limbal stem cell deficiency (LSCD), a condition leading to decreased visual acuity, photophobia, and ocular pain. To restore the ocular surface in advanced stem cell deficient corneas, an autologous or allogenic limbal stem cell transplantation is performed. In recent years, the risk of secondary LSCD due to removal of large limbal grafts has been significantly reduced by the optimization of cultivated limbal epithelial transplantation (CLET). Despite the great successes of CLET, there still is room for improvement as overall success rate is 70% and visual acuity often remains suboptimal after successful transplantation. Simple limbal epithelial transplantation reports higher success rates but has not been performed in as many patients yet. This review focuses on limbal epithelial stem cells and the pathophysiology of LSCD. State-of-the-art therapeutic management of LSCD is described, and new and evolving techniques in ocular surface regeneration are being discussed, in particular, advantages and disadvantages of alternative cell scaffolds and cell sources for cell based ocular surface reconstruction.

Long-term Outcome of Limbal Stem Cell Transplantation for the Management of Total Limbal Stem Cell Deficiency due to a Chemical Burn
Purpose: To evaluate the long-term outcome of limbal stem cell transplantation (LSCT) for the management of total limbal stem cell deficiency (LSCD) due to chemical burn in Labbafinejad Medical Center (LMC). Methods: Records of patients with a history of severe (i.e. grade III and IV) chemical burns, who were referred to LMC and underwent LSCT, were reviewed and data including demographic chara...
متن کاملLimbal fibroblast conditioned media: A non-invasive treatment for limbal stem cell deficiency
PURPOSE Limbal fibroblasts are known to regulate the maintenance and differentiation of the corneal epithelium including the limbal epithelial stem cells. This study examined the effect of limbal fibroblast conditioned media in a mouse model of limbal stem cell deficiency. METHODS Limbal stem cell deficiency was created in C57/Bl6 mice by performing a limbus to limbus epithelial debridement. ...
متن کاملPotential Role of Induced Pluripotent Stem Cells (IPSCs) for Cell-Based Therapy of the Ocular Surface
The integrity and normal function of the corneal epithelium are crucial for maintaining the cornea's transparency and vision. The existence of a cell population with progenitor characteristics in the limbus maintains a dynamic of constant epithelial repair and renewal. Currently, cell-based therapies for bio replacement-cultured limbal epithelial transplantation (CLET) and cultured oral mucosal...
متن کاملVariations of the Normal Human Limbal Stem Cells Detected by In Vivo Confocal Microscopy
Background To report normal variations of the limbal structures using in vivo laser scanning confocal microscopy. Methods: This was a retrospective study of fourteen eyes from 11 healthy individuals. Confocal imaging of cornea and limbus was performed using Heidelberg Retina Tomograph III Rostock Corneal Module. Results: The typical structure of the palisades of Vogt (POV) was detected ...
متن کاملThe Ocular Surface and How It Can Influence the Outcomes of Keratoprosthesis
Severe ocular diseases may result in partial or complete limbal cell deficiency. Besides conservative options, treatment options include conjunctival replacement procedures and limbal autografting. Limbal allografts are an option in patients with bilateral limbal cell deficiency. In many of these cases, a keratoprosthesis (KPro) is the last option to restore functional vision in patients with s...
